New Patient Registration
Anyone asking to register with the practice must live within our practice area. The list is open.
When anyone asks to register, reception staff will check your address against the map in the reception office.
If you live within the area, you will be asked for your old medical card or to complete form GMS1, which will need to be fully completed so that we can obtain your medical records from your previous doctor.
As a result of guidance received from NHS Counter Fraud Services all new patients registering with this practice with effect from 1st May 2015 will need to provide proof of identification.
This guidance relates to patients who register as permanent patients and also patient who register temporarily.
The following can be accepted as identification BUT one item must be a Photo ID together with one document containing the address the patient is living at:
Driving Licence
Passport
Birth Certificate
Marriage Certificate
Medical Card
Local Authority Rent Card
Paid Utility Bills
Bank/Building Society cards/Statements
National Insurance Number Cards
Payslip
Letter from Benefits Agency/Benefit Book/Signing on Card
Papers from the Home Office
P45
We will NOT accept the following documents:
Library Card
Video Rental Card
Health Club Card
Private Rent Book
When you register, you will be registering with the practice as a whole.
All new patients who register with the practice will be offered the opportunity to participate in a new – patient health check.
